fall, students are given “emergency cards” to complete and return to
school. These cards help us identify students who take medications
and/or have special health needs. In the event of illness or injury,
the information is used to contact parents or guardians.
Remember: A parent signature is required. Also included for
parents to complete is an authorization form for the exchange of
health and education information called the Health Insurance
Portability & Accountability Act (HIPAA).
This authorization assists Student Health personnel to communicate
with physician offices when necessary.
